Important Notice about Your Basic Group Term Life and Work-Related Accidental Death Policies Provided by the University
You will be receiving a letter advising you that effective January 1, 2013, the Metropolitan Life Insurance Company (MetLife) will provide your Pennsylvania State System of Higher Education Basic Group Term Life and Work-related Accidental Death Insurance coverages since this coverage is provided through participation in the Commonwealth of Pennsylvania’s Group Life Program.
This letter advises that you can visit www.metlife.com/mybenefits to review and update your beneficiary designation(s). To log-on, you will need to use “Commonwealth of Pennsylvania or Commonwealth of PA” as your employer. Do not use Mansfield University. You will also need to use your social security number and date of birth to register a unique User ID and Password, which you will use to access the site. If you do not see your beneficiary information or the beneficiary information is not current, it is not necessary to call MetLife and request a form. You can designate a beneficiary through the above MetLife website and print a copy for your records. A confirmation that a beneficiary was named will be mailed to you by MetLife; however, the designated beneficiary or beneficiaries is not included on the mailed confirmation.
If you do not have access to the internet or if you do not wish to use MetLife’s website, you may call MetLife at the number below to confirm your current beneficiary or to request a form to update your beneficiary designation.
